In any case, it’s important to know that this is all part of the care journey, and that there are experienced people who can help you understand the care you need, as it can be different for everyone.

Aged Care Assessment teams are made up of a mix of health and wellbeing professionals, ranging from medical officers and social workers to nurses, occupational therapists and physiotherapists.

Someone from your aged care assessment team will visit your home, discuss your care needs, and help you know which of the wide range of government-funded care services such as home and community care, respite care and residential aged care will suit you best

  1. Start by contacting My Aged Care (the Federal Government) on 1800 200 422. They will discuss your situation with you, and let you know if you will need to contact an Aged Care Assessment Team.
