Empowered Living Community Care

Empowered Living Community Care is a registered provider of Community Aged Care Packages. Once contacted, the Empowered Living Community Care Coordinator will make an appointment to visit the Client at home to discuss their individual care requirements. Then, in collaboration with the client and their representative, a tailored program is developed – one that not only establishes the best method of delivering their identified care needs, but that also has a genuine understanding of the cultural values and lifestyle of the client. Case management is a collaborative process of assessment, planning, facilitation and advocacy, which ensures that each individuals health needs are met and ensures high quality, coordinated and cost effective outcomes.
