Through our CHT Home Care Service, we offer an extensive range of in-home services that are purpose designed to your needs and help you achieve the lifestyle you want to achieve. If you have had an assessment by and approval from the Aged Care Assessment Team (ACAT), you qualify to receive our home care services. Our home care services are provided under the Consumer Directed Care model which ensures you are consulted and involved in selecting the services you need, the services you want, the best time for them to be delivered and the cost of those services.
