We strive to maintain each person’s lifestyle and independence and we do that by listening and providing choice and flexibility. EACH Home Care Packages are an individually planned and coordinated suite of supports and services designed to assist older individuals to live comfortably in their home. We design your package based around your unique care needs. The types of services that may be provided as part of a package include: • Case Management Support; • Advocacy; • Basic garden and home maintenance; • Assistance with personal care; • Nursing care, including medication management; • Home cleaning; • Meal preparation/support; • Transport to appointments and social support; • Exploring respite options. Home Care package level 2
Low-level care
Personal care, household or domestic assistance, social support, meals or shopping, or rehabilitation. Services may include loan of basic equipment.
$34,174.95 package
Home Care package level 3
Mid-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
$51,808.10 package
Home Care package level 4
High-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
