Ballarat Health Services (Linkages / Ballarat Aged Care) delivers a range of services to support your choice to remain living at home safely and with confidence. Services are designed to promote your independence, wellness and enhance your quality of life as an older person residing in the community. Ballarat Health Services is an Approved Provider of Home Care Packages: Level 1 - Basic care / Level 2 - Low level care / Level 3 - Intermediate care / Level 4 - High level care. All Home Care Packages are delivered within a Consumer Directed Care framework, which means you (and your carer) have more choice and control over what services are delivered and where and when they are delivered.
