If you need support to remain in your own home, you may be eligible for a government subsidised Home Care Package. A Home Care Package (HCP) abides by Consumer Directed Care (CDC), which is a model of service delivery designed to give more choice and flexibility to consumers. Consumers who receive a HCP have more control over the types of care and services they access and the delivery of those services, including who delivers the services and when.
