An experienced Case Manager will meet with you in person, understand your individual circumstances and be available if you have any questions. They will be respectful of your wishes in assisting you to develop a plan for services and supports that will enable you to stay at home as long as possible. As a not-for-profit organisation, our interest is in delivering choice and quality of services to you.
