Southern Plus: South West Home Care Services is an approved provider for Home Care Packages, subsidised by the Australian Government.
Location
Southern Plus based home care services covering the 300km2 coastal area encompassing Bunbury, Margaret River, Busselton and Augusta. We offer both HACC and Home Care Packages in the region to help you stay at home for as long as possible. Our Home Care Packages allow you to select from a variety of services including assistance with household tasks, nutrition, shopping, centre based day care, nursing care, allied health, respite for carers, podiatry and personal care. Home Care Packages can also provide access to our specialist dementia and clinical care team if they are needed. Our services recognise the differing needs of people from culturally and linguistically diverse backgrounds, including Aboriginal and Torres Strait Islander communities. $15,705.95 package
Home Care package level 2
Low-level care
Personal care, household or domestic assistance, social support, meals or shopping, or rehabilitation. Services may include loan of basic equipment.
$34,174.95 package
Home Care package level 3
Mid-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
$51,808.10 package
Home Care package level 4
High-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
